LECTURE & HANDS - ON WORKSHOP

BIOLOGICAL APPROACH TO IMMEDIATE IMPLANTATION

TALLINN | 22-23 MAY 2026

PROGRAM

Theoretical part

Lecture – Dr. Natalja Kull

Effective Alveolar Ridge Augmentation: Biological Principles and Practical Application

Efficient alveolar ridge augmentation is often a prerequisite for successful dental implantation. While autogenous bone remains the gold standard, modern bone substitute materials are increasingly competing alternatives.
In today’s era of biological augmentation, new possibilities have emerged for the targeted and predictable use of autogenous bone in a variety of clinical situations. This approach enables minimally invasive and biologically optimized treatments, ensuring stable and long-term results.
The lecture focuses on the clinical application of the Khoury technique in different scenarios and provides participants with a comprehensive overview of the biological bone augmentation (BBA) principles and their practical execution.

Lecture – Dr. Krasimir Zlatev

Lecture: IDR – Biological approach to immediate implantation

  • Promotion of Biological decision three
  • Indication for IDR
  • Comparison between classic workflow and IDR protocol
  • Extraction socket classification and decision making - literature review
  • Requirements to the implant
  • Classification of alveolar bone defects by Dr. Jose Carlos Da Rosa, application.
  • Main principles of IDR Technique
  • Biology of maxillary tuberosity bone graft
  • CBCT with soft tissue contrast
  • Backward planning. Utilization of digital protocols
  • Evaluation of the surgical site: On ST CBCT, PA, Clinically before and after minimally invasive tooth extraction
  • Minimally invasive tooth extraction- tools
  • IDR definition
  • Minimally invasive tooth extraction- one root tooth, multiple root tooth
  • Debridement and disinfection protocol
  • Bone graft harvesting from maxillary tuberosity-tools
  • Bone graft harvesting from maxillary tuberosity-different kind of grafts, flap design
  • Bone harvesting: Alternative approaches, tools and sites
  • Implant size selection, 3D positioning
  • Osteotomy preparation
  • Stabilization of the graft into the socket
  • Mapping of bone defect
  • IDR and osseodensification (OD)
  • Sinus crestal approach with Versah protocol in IDR cases
  • Intra operative prosthetic stage

Every step will be explained with clinical pictures and surgical video.

Case presentation

  • Implants premolar zone ( upper and lower jaw)
  • Implants molar zone ( upper and lower jaw)
  • Single implants esthetic zone
  • Multiple implants esthetic zone
  • Implants in socket with chronic infection ( periapical granuloma)
  • Implant in compromised socket due to old root fracture ( infected site)
  • Implant in socket with active infection ( fistula)
  • Combination: ortho extrusion technique for regeneration of perio bony lesion and IDR
  • IDR in perio patient
  • Complication management

Each case is documented with clinical pictures before, after and during operation. For most of the cases there is surgical video. CBCT before, in the OP day and 4 months after, or longer period of time will demonstrate the volume of buccal bone and soft tissue. Depends of time available, between 13 and 20 clinical cases can be shown.

Hands-on Workshop: Pig jaw training

1. Intact socket case

  • Atraumatic tooth extraction
  • Implant placement
  • Bone harvesting from the tuberosity using a straight chisel and surgical hammer
  • Grafting with crushed autogenous bone
  • Placement of provisional crown and temporary Ti abutment

2. Compromised socket case

  • Creation of buccal wall defect
  • Implant installation and bone defect evaluation
  • Bone harvesting with calibrated concave chisel
  • Shaping and adapting the cortico-cancellous graft
  • Grafting and socket sealing with custom-made provisional abutment
